Empirical research is the foundation of scholarly research and scholarly writing.

Empirical research is the foundation of scholarly research and scholarly writing. An empirical article is defined as one that reports actual results of a research study. An empirical article includes a description of the study, an introduction, a research question, an explanation of the study’s methodology, a presentation of the results of the study, and a conclusion that discusses the results and suggests topics for further study.

As you search the library for scholarly research, you should limit your search to identify empirical articles. (You can use the “Empirical Research Checklist” from DNP-801 to assist in this determination.) After finding an empirical study, begin to assess the validity of the conclusion by determining if the conclusion answers the proposed research question and if the methodology is appropriate.

As you move forward in your doctoral journey, you will read research papers that will require you to assess the validity of the studies in question. To accomplish this, qualitative assessments about the research must be made by comparing, contrasting, and synthesizing what the research says.

In this assignment, you will continue to develop the skill of writing, by reviewing 15 research articles associated with your chosen topic and using the content to identify at least five major concepts and subthemes related to your topic.

General Requirements:

Use the following information to ensure successful completion of this assignment:

Use the “Empirical Research Checklist” to assist in the determination of empirical articles.

Use the “Research Article Chart” to provide a summary review of each component of your assignment.

Doctoral learners are required to use APA style for their writing assignments. The APA Style Guide is located in the Student Success Center.

This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion.

You are required to submit this assignment to Turnitin. Please refer to the directions in the Student Success Center.

Directions:

Part 2: Research Article Chart

Using the articles acquired in Part 1, provide a summary review of each component using the “Research Article Chart” template.

Part 3: Synthesis of Research Studies

Based upon your review of the 15 research articles, identify at least five major concepts that relate to your project topic, and three subthemes related to each concept.

Using the information from the completed “Research Article Chart,” and the major concepts and subthemes you have identified, write a 2,000-3,000 word paper that synthesizes the content of the 15 research studies.

Summarize each major concept and focus on providing a detailed synthesis of the three identified subthemes that support that concept by addressing the following.

Introduction of the identified subtheme

Summary of the research questions posed by the studies

Summary of the sample populations used

Summary of the limitations of the studies

Summary of the conclusion and recommendations for further research

The nursing shortage can have adverse impacts to patients’ health-care outcomes.

The projected nursing shortage can incur significant impact to the nursing profession and to the public. For one, the anticipated increase of the U.S aging population, this will put more strain on the nursing profession to find nursing to care for this population. At the same token, there is a great proportion of the nursing workforce that will reach retirement age. This will create an imbalance situation in terms of nurse-to-patient ratios. Such event will indubitably trigger anxiety, job burnout, turnover, absenteeism, and low morality within the nursing profession.

The nursing shortage can have adverse impacts to patients’ health-care outcomes. The public might suffer considerably from medical errors; nurses may have less time to spend and care with patients. As a result, the proportions of hospital readmission and patient death can increase exponentially.

The nursing profession will face with the urgency to provide answers to the nursing workforce disparity in relation to the growing aging population. Such ageing population are suffering from chronic diseases that require long-term care. That is, the nursing profession must increase the nursing workforce pool to compensate for the projected shortage.

One way that the nursing profession is dealing with resolving this issue is through public-private partnerships. Such a strategy can put emphasis on encouraging nurses to access to higher education so that they become nurse educator. This will require providing incentives to nurse. In the same perspective, efforts should be done to launch advertising programs that show positive aspects of the nursing profession. Coupled with these, community leaders, healthcare associations, and Nursing Association should advocate elected officials to provide more funds to help the nursing profession.

Comment 2

The nursing shortage is not only related to the “baby boomers” retiring. It is also the qualified nurse not willing to work in the present conditions. These conditions are inadequate workforce planning, understaffed, poor recruitment, retention, return policies, and the ineffective availability of nursing resources. High patient to nurse ratios is causing the nurses today to have “burn out” and they eventually leave their nursing career. The failure to deal with the nursing shortage will lead to the failure to maintain and improve health care and it will have a huge impact on the nursing profession and the public. Addressing the nurse shortage and providing a more positive staffing environment in which the clinical practice can actually be delivered is a resolution to the nursing shortage. Providing a more positive staffing environment includes motivating the nurses, providing support, really looking at patient acuity and nurse to patient ratios, providing incentives on nurse retention and nurse recruitment. Nurses should be able to have the opportunities to develop professionally, have opportunities for growth, gain autonomy, and have a say in the decision making and truly be listened to by their managers and administration team. (Buchan & Aiken, 2014). Most individuals become nurses because they truly care and have compassion. They want to help people get better, this includes on their job along outside their jobs. Nurses take their profession very serious but there is only so much one person can do. Nurses have families also and should be able to come into work and take care of their patients in a safe environment and go home to spend time with their families at the end of the day.

Comment 3

The nursing profession is at risk of an increased shortage of nurses. The increased shortage of nurses can strongly impact health care corporations. In my current position in both the Nursing home/Rehab and the hospital setting, I have noticed that when we have shortages nurses are placed at risk for harm. Nurses become responsible for increased workload, which can lead to physical and emotional stress. This places both you and your patients at danger.

Another issue that arise within my hospital when short staffed is the ability to admit patients. If we have reached a full census we are unable to admit patients from the emergency room. This will place the emergency room on full capacity, meaning patients will need to go to the nearest hospital. This potentially can delay care for patients that urgently need it. Management also prefers that we keep all of our beds full to ensure maximum revenue and with out the appropriate staff we are unable to do so. The nursing shortage is quickly approaching. According to The Health Resources and Services Administration “more than 1 million registered nurses will reach retirement age within the next 10 to 15 years” (AACN, 2014). This will create a major set back reinforcing the issue stated above. Many organizations are working on hiring young graduate students in order to replace the baby boomers class. This provides a company and the nursing community with longevity in an attempt to replace the baby boomer class. Many states are working to expand the nursing programs as well as create more in order to keep up with the current and up and coming shortage.

New Models For Evidence-Based Practice

A professional goal for DNP-prepared nurses is to produce new evidence-based models of care and develop evidence-based guidelines. As you continue to develop and engage in your EBP Project, keep this aim in mind. What could help to facilitate the acceptance of new practices?

As noted in earlier weeks, every specialty area should base practice on research and evidence. For example, in informatics, an informatician who plans to implement a new health information system would be wise to follow a model that research shows has been effective. Similarly, those involved in leadership or management can review change models and theories, exploring current evidence on best practices that produce positive results. For this Discussion, you evaluate evidence-based practice models, change theories, and frameworks that assist with the adoption of new evidence-based practice approaches.

To prepare:

Reflect on your hospital setting and the use of evidence-based practice models. How are these models used? If they are not used, how might the integration of an evidence-based practice model change practice?

Review the Rosswurm and Larrabee article presented in the Learning Resources focusing on the model they present for integrating new evidence-based practices (See attached file).

Research and locate an evidence-based practice model from nursing or another discipline that could be used in your specialty area to lead a systematic change to evidence-based practice approaches. You may also use a model or framework discussed in your course text, Translation of Evidence into Nursing and Health Care Practice.

Consider how using a model for evidence-based practice can facilitate adoption of new approaches to practice.

By tomorrow Tuesday 9/25/18 10 pm, write an essay of a minimum of 550 words in APA format, and at least 3 scholarly references from the list of required readings below. Include all level one headings as numbered below:

Post a cohesive response that addresses the following:

1) Discuss the use of evidence-based models in your hospital setting.

2) Describe a model for evidence-based practice or change theories and frameworks that you could utilize to facilitate the adoption of new approaches to practice. Substantiate your selection with evidence from the literature.

Evidence based practice is essential to developing clinical information systems in order to further the generation of nursing knowledge.

Without the various roles and expertise of professional nurses in the development of nursing informatics, clinical information systems would not be able to have a foundation based on evidence-based practice. Evidence based practice is essential to developing clinical information systems in order to further the generation of nursing knowledge. Evidenced based practice is a process that uses the current best evidence in the care of patients as an approach to problem solving (McEwen & Wills, 2017). By nature, nurses gather data and record and collect clinical data that is objective on a daily basis. Professional nurses will gather this clinical data and identify trends seen across groups of patients (McGonigal & Mastrian, 2017). A solution to a problematic trend may be identified, implemented and seen to improve or make a positive impact on such trend which would be an example of evidence-based practice.

The use of nursing informatics through the electronic medical record, which is one of the most widely used clinical information systems in nursing today, has been heavily dependent on evidence-based practice. The EMR can be broken down to professional nurses chronicling their daily clinical challenges and queries (McGonigal & Mastrian, 2017). Documenting in an EMR includes using clinical practice guidelines which have been generated through evidence-based practice. For example, a patient can be identified as a fall risk using a basic fall risk assessment that has been previously synthesized to tailor specific criteria in identifying fall risks through daily nursing practice. This will usually would include age, previous fall incidence, medication, mobility and cognition. Once this fall risk has been identified, the nurse would open a clinical practice guideline parameter to document implemented patient centered care and focused interventions in the real-world setting (Vance, 2012). This is merely one example of knowledge brought into the CIS through research conducted by nurses. Evidence based practice was founded only to be useful in integrating into clinical information systems when is it used in clinical workflow (Bakken, Currie, Lee, Roberts, Collins & Cimino, 2008). Hence, the implementation of the CPG driven support system. Every clinical practice guideline has been developed through the use of evidence-based practice and then brought into the clinical information system of EMR.

The use of an info button is another example of evidence-based practice incorporated into CIS. The primary advantage of the info button approach from the clinician perspective is that the information provided is context-specific (Darvish, Bahramnezhad, Keyhanian & Navidhamidi, 2014). Info buttons actually serve as an additional source of evidence-based practice which can be accessed within another clinical information system, such as an EMR. They can be used to determine critical lab results and possibly their perspective treatments or they can be used to gain access to medication inquisitions. In both instances, the use of evidence-based practice is the foundation of the knowledge and information used.

Incorporating the use of informatics through clinical information systems such as electronic health records and info buttons allows for information to be computed on a daily basis. Through research, nurses become experts on analyzing and synthesizing knowledge and furthermore put the knowledge to practice with patients and family members. Clinical information systems are a vessel of nursing evidenced based practice put into clinical workflow which allows the continuous and further generation of nursing knowledge. Since clinical information systems are used widely throughout the nursing practice from bedside nurses, nurse practitioners to nurse researchers, it is imperative that the information being provided by such systems stay evident and factual, through evidence based practice.

Assessing the Ears, Nose, and Throat

Most ear, nose, and throat conditions that arise in non-critical care settings are minor in nature. However, subtle symptoms can sometimes escalate into life-threatening conditions that require prompt assessment and treatment. Nurses conducting assessments of the ears, nose, and throat must be able to identify the small differences between life-threatening conditions and benign ones. For instance, if a patient with a sore throat and a runny nose also has inflamed lymph nodes, the inflammation is probably due to the pathogen causing the sore throat rather than a case of throat cancer. With this knowledge and a sufficient patient health history, a nurse would not need to escalate the assessment to a biopsy or an MRI of the lymph nodes, but would probably perform a simple strep

In this Discussion, you consider case studies of abnormal findings from patients in a clinical setting. You determine what history should be collected from the patients, what physical exams and diagnostic tests should be conducted, and formulate a differential diagnosis with several possible conditions.

Note: By Day 1 of this week, your Instructor will have assigned you to one of the following specific case studies for this Discussion. Also, your Discussion post should be in the Episodic/Focused SOAP Note format, rather than the traditional narrative style Discussion posting format. Refer to Chapter 2 of the Sullivan text and the Episodic/Focused SOAP Template in the Week 5 Learning Resources for guidance. Remember that all Episodic/Focused SOAP notes have specific data included in every Case 2: Focused Throat Exam

Lily is a 20-year-old student at the local community college. When some of her friends and classmates told her about an outbreak of flu-like symptoms sweeping her campus over the past two weeks, Lily figured she shouldn’t take her three-day sore throat lightly. Your clinic has treated a few cases similar to Lily’s. All the patients reported decreased appetite, headaches, and pain with swallowing. As Lily recounts these symptoms to you, you notice that she has a runny nose and a slight hoarseness in her voice but doesn’t sound congested. patient case.test.

o prepare:

With regard to the case study you were assigned:

Review this week’s Learning Resources and consider the insights they provide.

Consider what history would be necessary to collect from the patient.

Consider what physical exams and diagnostic tests would be appropriate to gather more information about the patient’s condition. How would the results be used to make a diagnosis?

Identify at least 10 possible conditions that may be considered in a differential diagnosis for the patient.

Note: Before you submit your initial post, replace the subject line (“Week 5 Discussion”) with “Review of Case Study ___,” identifying the number of the case study you were assigned.

Laying the Foundation for New Approaches to Practice

As frequently noted in the literature, there is often a gap between research evidence and practice. This is not a new phenomenon; however, with the rapid growth of knowledge, it is essential that nurses hone their ability to identify this gap, determine best practices based on the evidence, and then apply this evidence to practice through the development of new approaches. In order for that to transpire, nurses must be skilled in leading change efforts so that adoption of new practice approaches can be successful.

For your EBP Project in Week 2 (Application 1 [SEE ATTACHED FILES] ), you identified an issue in your practicum setting in which the outcome is different from what would be expected according to the research literature. This week continue to explore current research literature looking for new evidence to address the problem. In addition, you build on the work you began in this week’s Discussion and select an evidence-based practice model ( The Iowa Model of Evidence-Based Practice to Promote Quality Care” Or you can use the Johns Hopkins Model) and change theory (USE THE THEORY OF PLANNED BEHAVIOR) or framework that will support your efforts.

To prepare for Application 2 address the following:

How current is the science underlying the practices related to your EBP Project issue?

Consider possible causes of the difference in outcomes from what would be expected according to the research literature.

Evaluate the effect the EBP Project issue is having in practice.

Using the Walden Library and other credible sources, locate current research evidence of new practices that are effective in addressing the issue.

Use the GRADE model presented in the Guyatt et al. (2011) article to evaluate the research evidence (see attached file for the article)

The full Application 2 is due by Day 5 of this week. Instructions for how to prepare for previous sections of this Application have been provided in Week 4.

Informatics In Healthcare

Best Hospital is a 325-bed suburban community hospital that has just merged with a local major academic medical center, the University of Excellence Medical Center (UEMC). There are 15 hospitals in the UEMC system, ranging from small rural hospitals with 50 or fewer beds to large multiple-specialty tertiary referral hospitals. Two hospitals in the system have achieved magnet status and several hospitals are working to achieve this status. Within the UEMC system, a high value is placed on collaboration with the schools of the health professions at the University of Excellence (UE).

Best Hospital has a long history of association with local universities, functioning mainly as a site for student clinical experiences. For example, undergraduate students enrolled at the UE College of Health Professions often complete a portion of their clinical work at the hospital. Each year the hospital hires a number of graduates from this program. Several nurses in first-level management positions at the hospital are enrolled in the master’s program.

The vice-president for clinical practice at Best Hospital is a PhD-prepared nurse who has been appointed to the faculty at the UE School of Nursing. Your current position is director of staff development at Best Hospital. You are responsible for the educational programming of all clinical staff. Over the past 2 years, Best Hospital has been fully involved in meeting Meaningful Use criteria. As director of staff development, you have played the major leadership role in developing and implementing all related educational programs.

The health professional schools at UE are working together to revise their curricula to integrate informatics throughout. The goal is to develop an interprofessional approach that incorporates informatics across and throughout the different educational programs within these schools. The schools have requested a meeting with clinical leaders from the hospital to gain a practical perspective on what information should be included in the curricula. The vice-president for clinical practice has included you on the list of leaders to attend the meeting. She asked that each person attending the meeting prepare a short presentation on how they engage their staff in the continuous education process with respect to ever-changing healthcare technology.
