Benchmark – Individual Client Health History and Examination

In this assignment, you will be completing a health assessment on an older adult. To complete this assignment, do the following:

  1. Perform a health history on an older adult. Students who do not work in an acute setting may “practice” these skills with a patient, community member, neighbor, friend, colleague, or loved one. (If an older individual is not available, you may choose a younger individual).
  2. Complete a physical examination of the client using the “Health History and Examination” assignment resource. Use the “Functional Health Pattern Assessment” resource as a guideline to assist you in completing the template.
  3. Document findings of complete physical examination in Situation-Background-Assessment-Recommendation (SBAR) format. Refer to the sample SBAR Template located on the National Nurse Leadership Council website at https://www.ihs.gov/nnlc/includes/themes/newihstheme/display_objects/documents/resources/SBARTEMPLATE.pdf as a guide.
  4. Document the findings of the physical examination in the assessment worksheet.
  5. Using the “Health History and Examination” assignment resource, provide the physical examination findings summary with planned interventions for the client. Include any community services in the interventions.

It becomes very challenging on an individual as the aging process begins. Although many individuals age comfortably and remain active throughout the life span, others may experience the effects of medical conditions, cognitive disorders, psychological and spiritual issues. As health care workers it is important to know what to expect and how to help individuals to take steps to counterbalance the effects of aging to maintain as much independence as possible. The following are all issues that the aging adult may deal with.

What is a research proposal?

The purpose of a research proposal is to identify and formulate questions that arise from nursing practice and to design a research project to study the identified problem. The goal of a research proposal is to present and justify a research idea you have and to present the practical ways in which you think this research should be conducted. Research proposals contain extensive literature reviews and must provide persuasive evidence that there is a need for the proposed research study. In addition to providing a rationale for the proposed research, a proposal describes detailed methodology for conducting the research consistent with requirements of the professional or academic field and a statement on anticipated outcomes and/or benefits derived from the study.

Directions

Write a research proposal using your clinical question formulated earlier in the course that was approved by your instructor. This may be either a qualitative or quantitative research proposal. Your proposal must include APA formatting. The proposal should be a 6–10 in length plus appendices cover page, and reference page. The appendices should include your survey instrument created in week 6. You will be presenting your research proposal in a 6–8 minute recorded presentation in week 8.
