This article stresses the necessity of sufficient safeguards for EHR systems. The author explores current safety regulations for EHR system design and deployment. The author makes additional recommendations for protecting public health in the digital area.

Discussion: Security of Health Care Records

With the increase of health information technology used to store and access patient information, the likelihood of security breaches has also risen. In fact, according to the Canadian Medical Association Journal (CMAJ):

In the United States, there was a whopping 97% increase in the number of health records breached from 2010 to 2011… The number of patient records accessed in each breach has also increased substantially, from 26,968 (in 2010) to 49,394 (in 2011). Since August 2009, when the US government regulated that any breach affecting more than 500 patients be publicly disclosed, a total of 385 breaches, involving more than 19 million records, have been reported to the Department of Health and Human Services.

A large portion of those breaches, 39%, occurred because of a lost, stolen, or otherwise compromised portable electronic device—a problem that will likely only get worse as iPads, smartphones, and other gadgets become more common in hospitals. (CMAJ, 2012, p. E215).

Consider your own experiences. Does your organization use portable electronic devices? What safeguards are in place to ensure the security of data and patient information? For this Discussion you consider ethical and security issues surrounding the protection of digital health information.

To prepare

  • Review the Learning Resources dealing with the security of digital health care information. Reflect on your own organization or one with which you are familiar, and think about how health information stored electronically is protected.
  • Consider the nurse’s responsibility to ensure the protection of patient information. What strategies can you use?
  • Reflect on ethical issues that are likely to arise with the increased access to newer, smaller, and more powerful technology tools.
  • Consider strategies that can be implemented to ensure that the use of HIT contributes to an overall culture of safety.

Z tests and t tests are formulas used in hypothesis testing.   Hypothesis testing is a way to figure out if results from a test are valid and repeatable.   Z tests are used when your data is normally distributed, the standard deviation is known, and the sample size is greater than 30. T tests compare the two means of tests and tells you if they are different from each other and how significant the differences are, meaning could they have happened by chance.   T tests are more commonly used and do not require the standard deviation be known.

Qualitative research gathers information that is not in numerical form.  Qualitative data is typically descriptive data and as such is harder to analyze than quantitative data

Qualitative research is useful for studies at the individual level, and to find out, in depth, the ways in which people think or feel .

Researcher can manage and organize the data by starting by developing a clear, meaningful file-naming system, such as the collection method, the date and the collector’s initials. Make this the standard for naming all qualitative data files. Then, create standard operating procedures for transforming raw data into useful information. This typically involves transcribing written data into an electronic format, categorizing and coding relevant data to find recurring themes or patterns and reviewing information to make sure it is correct. Be mindful as you go about creating schedules and setting due-dates that organizing qualitative data is often a time-consuming process .Analysis and interpretation attach significance to the information the data provides. In this step, you make sense of your results, look for underlying causes that explain results and draw conclusions. The goals are to confirm what you may already know, uncover misconceptions and identify important things that you didn’t previously know, but that you should know. For example, in analyzing results, you might confirm that communication patterns between employees and managers need improvement, discover department managers aren’t following communications protocols and that the general perception is that employees feel certain managers show favoritism.

Analysis of data requires examining, sorting, and reexamining data continually. Qualitative researchers use many means to organize, retrieve, and analyze their data. Many researchers simply use notebooks and boxes of paper. As of researcher Bogdan and Biklen (1992) describe what they call two mechanical means to organize and begin to review data. One way they describe is to write initial codes in margins of field notes, photocopy the notes and store the originals, then cut up and sort the text segments into piles according to codes. These coded data can be stored in boxes and resorted and analyzed on an ongoing basis. A second method they describe is to record field notes on pages on which each line is numbered, code the field notes, and then write the page number, line numbers, and a brief description of each piece of data on a small index card. These cards can then be sorted and analyzed. The authors note that this second method is better suited for small sets of data, as it often requires returning to the original field notes to analyze the actual data.

Food Intake: 3 Days

Record food intake and activities performed for three days. The iProfile® will be utilized to analyze the 3-day Food Intake Record. The Activities data will be analyzed during Week 4.

Write a paper of at least 750 words that addresses the following points about your 3-day food intake:

  • Recorded intake of protein, carbohydrates, and lipids
  • Which foods in your recorded daily intake provide protein? Which provide carbohydrates? Which provide lipids?
  • Review how your recorded protein, carbohydrate, and lipid intake compares with the recommendations of the dietary reference intake. If your recorded protein-carbohydrate-fat intake was too high or too low, which foods might you add or remove to achieve your goal and keep other nutrients in balance?
  • Is the protein in each food you ate complete or incomplete, combining to become complementary? Why is this important?
