A multitude of different social, economic and cultural factors determine a person’s health. This means that people living in the same community, or people of the same age, can have vastly different chances of good health.
Instinctively you might think that access to health services is the biggest factor in determining health. However, many other things significantly affect our likelihood of good health. It can also be easy to blame individuals for their poor health, or congratulate others for good health, yet in reality many of the determinants of health are out of our own personal control. Underlying these broad factors is a much deeper level of structural causes, which are more difficult to change.
John is 30 years old and has a history of asthma, diabetes, is a smoker and has been overweight for most of his adult life. He lives with his wife and 3 children on a council estate housing which is overcrowded. Often he is unable to pay for the electrical bill and provide food for his family. His wife and children are overweight and they suffer from asthma. John is due to go back to his country as his visa to remain in this country will expire soon. He recently went to the GP after not visiting the GP for 2 years about a minor complaint, and as a matter of course his blood pressure was taken. Much to his surprise, it was above what was considered acceptable. His doctor started him on medication because his high blood pressure was high enough to warrant immediate action.
His blood pressure dropped, but unfortunately he still did not change his lifestyle and did not take his medication as scheduled. John is unemployed for the past 2 years and receives unemployment benefits so he does not have very much disposable income. John subsequently became depressed and lacks energy. He returned to his GP, who sent him to a Health and Wellbeing Clinic to check his blood pressure and to seek advice from health professionals who would give him advice on how to better control his blood pressure, diabetes and overweight. At the clinic he was given advice on how to change his lifestyle, which included reducing salt in his diet. However, due to cultural factors, socioeconomic factors, he struggled to eat proper meals and he did not follow any of the advice given and his blood pressure and diabetes remained a cause for concern.

According to Rouse (2018), health informatics is a new technological practice that is currently used in health centers for acquiring, studying, and managing health information and data to ensure that better healthcare services have been achieved. Here, the role of technology and its integrated sciences been effectively applied in the nursing field. Notably, informatics has been a fast-increasing tool as technology advances over time.

Undeniably, informatics is profoundly applied in many areas including education and in providing healthcare services. In the healthcare field, informatics is mostly used in differentiated backgrounds including public health, pharmacy, dentistry, computer science, and improving the quality of life. However, in the field of nursing, informatics is used in maintaining the electronic health records (EHR), designing and training other on how to use this new system, acting as a consultant in this field, developing new policies within the nursing field, and managing outcomes that result from the healthcare activities.

Although informatics plays a vital role in the nursing field, the most use of informatics in my field is electronic recording. According to Lown and Rodriguez (2012), electronic record involves documentation of patient’s information through a comprehensive patient care system in electronic devices mainly for medical purposes only. Unlike other methods of recording data, the EHR data recording involves recording information about patients on an online platform which is than entrusted to nurses to handle that data. Here, the information that is received in the EHRs is then used when needed and can be shared from one facility to the other and from one physician to another (Häyrinen, Saranto & Nykänen, 2008). Notably, EHRs holds various benefits to nurses. For instance, the EHRs comes with new improvements such as increasing the facility’s productivity. Moreover, this system is faster and timely, cheap, accurate, competent, secure, easy to use, and provides up to date information which gives reasons why it is profoundly recommended (Hoffman, 2016).

One of the primary areas where I can apply EHR is in the pre-hospital care. An excellent example of pre-hospital care use of EHR is the ambulance services (Koufi, Malamateniou, & Vassilacopoulos, (2010). Here, nurses would be feeding in data and physical conditions of the patients before they reach to the hospital. In these ambulances, the role of EHR would be to ensure patient data sharing, injury /illness prevention, and designing of treatment methods. In conclusion, if nurses can effectively apply the informatic tool, they can lead to a developed health care service.

While HIV/AIDS is still currently incurable, the prognosis for patients with this infectious disease has improved due to advancements in drug treatments. Consider the case of Kristy Aney. You are the manager of a new primary care clinic located about twenty five (25) miles outside of a small city (population of 50,000). With five (5) family physicians, two (2) nurse practitioners, two (2) physicians’ assistants (PAs), and twenty (20) clinical support staff consisting of RNs, LPNs, and CMAs, the clinic provides primary care services to a diverse community of people living and working outside the city limits. The following is what you should consider for this assessment

a) Report Title – should include the health issue, target population
group and the area. London or New York based papers should look at

the boroughs; elsewhere you can just look at the whole city as one

study unit.

b) Introduction – brief overview of the urban health issue and
signposting to show the sections you will include in the work.

c) Rationale of why this is an urban health issue – identify and explain
the evidence including epidemiological data you will use to show why

this is an urban health problem.

d) Explain the urban context and determinants that you will look at to
explain the impact on the health issue. If you are looking at the same

topic and borough as before, you can draw on the Urban Health

Profile. Remember to draw on the wider context, including London if

you are looking at a London borough.

e) Identify and explain the public health consequences and
implications of this urban health problem that impact on the general

populations, on the individual and on the health services.
