How would you conduct a breast cancer needs assessment?

When you are engaged in program planning, a needs assessment helps to determine the actuality and details of a specific problem. It also addresses how the target population views the problem—which is critical since their perspective and motivation to change are essential for the success of the program.

In this week’s video, the necessity of appraising a community’s assets as well as its challenges is evident as Dr. Donna Shambley-Ebron and Dr. Rebecca Lee discuss needs assessments. What additional insights have you gleaned from the Learning Resources that you intend to apply to your program?

For this Discussion, you develop a framework for completing a needs assessment for your program. To do this effectively, it is essential to begin with the program and the population you have identified and then develop an appropriate approach for conducting a needs assessment. This also leads to consideration of the types of data needed and the data collection methods to be used.

To prepare:

Consider aspects of your problem and population (breast cancer in African American women in the USA) as you review the information on needs assessment presented in the Learning Resources.

How would you conduct a needs assessment? What considerations would influence your decision making about this process?

What would be the most efficient and effective methods for collecting data? What challenges could be encountered?

By tomorrow Wednesday 12/19/18 2 pm, write a minimum of 550 words essay in APA format with at least 3 references from the list of required readings below. Include all headers as numbered below:

Post a cohesive scholarly response that addresses the following:

1) Discuss how you would conduct a needs assessment for your selected problem and the target population. Why is this approach appropriate for the problem you have identified? Support your response with evidence from the literature.

2) Describe your proposed data collection methods and evaluate any challenges you might encounter.

Required Readings

Assessment and Planning in Health Programs

Chapter 1, “Assessment for Developing Programs and Interventions: The Big Picture”

Chapter 1 notes the importance of looking not just at needs but also the assets associated with the target population and environment; that is, assessing what needs to be addressed given the assets that are present.

Chapter 3, “Data Collection Strategies for Needs Assessments and Evaluations”

In Chapter 3, the authors examine data collection, a key element of needs assessments. They also indicate that it is wise to plan ahead and consider data collection strategies for evaluation at the same time.

“Needs Assessment” (featuring Dr. Rebecca Lee, Shiniche Thomas, and Dr. Donna Shambley-Ebron)

Discuss three areas in which health problems or barriers to health were identified

Understanding family structure and style is essential to patient and family care. Conducting a family interview and needs assessment gathers information to identify strengths, as well as potential barriers to health. This information ultimately helps develop family-centered strategies for support and guidance.

This family health assessment is a two-part assignment. The information you gather in this initial assignment will be utilized for the second assignment in Topic 3.

Develop an interview questionnaire to be used in a family-focused functional assessment. The questionnaire must include three open-ended, family-focused questions to assess functional health patterns for each of the following:

Values/Health Perception

Nutrition

Sleep/Rest

Elimination

Activity/Exercise

Cognitive

Sensory-Perception

Self-Perception

Role Relationship

Sexuality

Coping

Select a family, other than your own, and seek permission from the family to conduct an interview. Utilize the interview questions complied in your interview questionnaire to conduct a family-focused functional assessment. Document the responses as you conduct the interview.

Upon completion of the interview, write a 750-1,000-word paper. Analyze your assessment findings. Submit your questionnaire as an appendix with your assignment.

Include the following in your paper:

Describe the family structure. Include individuals and any relevant attributes defining the family composition, race/ethnicity, social class, spirituality, and environment.

Summarize the overall health behaviors of the family. Describe the current health of the family.

Based on your findings, describe at least two of the functional health pattern strengths noted in the findings. Discuss three areas in which health problems or barriers to health were identified.

Describe how family systems theory can be applied to solicit changes in family members that, in turn, initiate positive changes to the overall family functions over time.

Cite at least three peer-reviewed or scholarly sources to complete this assignment. Sources should be published within the last 5 years and appropriate for the assignment criteria.

Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion.

summarize the feedback shared by three nurse colleagues and discuss whether their impressions are consistent with what you have researched about health reform

As the country focuses on the restructuring of the U.S. health care delivery system, nurses will continue to play an important role. It is expected that more and more nursing jobs will become available out in the community, and fewer will be available in acute care hospitals.

Write an informal presentation (500-700 words) to educate nurses about how the practice of nursing is expected to grow and change. Include the concepts of continuity or continuum of care, accountable care organizations (ACO), medical homes, and nurse-managed health clinics.

Share your presentation with nurse colleagues on your unit or department and ask them to offer their impressions of the anticipated changes to health care delivery and the new role of nurses in hospital settings, communities, clinics, and medical homes.

In 800-1,000 words summarize the feedback shared by three nurse colleagues and discuss whether their impressions are consistent with what you have researched about health reform.

A minimum of three scholarly references are required for this assignment.

While APA format is not required for the body of this assignment, solid academic writing is expected and in-text citations and references should be presented using APA documentation guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

Since the inception of public health programs, vital statistics have been a critical factor in the effort to improve sanitation and to control disease. Today, mortality and morbidity data are used more generally to study trends and differentials in all kinds of causes of death and illness. The availability of mortality and morbidity statistics for small geographic units has contributed uniquely to the value of these data for epidemiologic investigations, surveillance, and informatics.

List and explain at least three of the five major challenges posed by current methods of collecting morbidity data with respect to data accuracy and data reliability.

Why it is important for those collecting morbidity data to be able to ensure confidentiality for respondents, and list the legislative and administrative requirements imposed on the National Center for Health Statistics (NCHS) with respect to data confidentiality.

Morbidity and mortality statistical reporting come from, in part, coded medical data transmitted through electronic data interchange (EDI) from Health Information Management Systems (HIMS) to PHIS. These data have all been standardized under HIPAA. Explain what it means to use coded data in data collections from the HIMS and/or PHIS

Assess how the new nurse leader position will have power and influence and impact patient outcomes

Write a 4–6 page impact report to senior leadership that identifies a nursing-related challenge, analyzes how it affects the organization from a nursing perspective, and details the new position.

Each organization has a unique structure that impacts the behavior of the organization. In health care organizations, this impact is on the ability of the organization to deliver quality patient care. Nursing is an essential component of any health care organization, and nursing leaders must understand the role of nursing and nursing leadership within the total organization. Understanding basic organizational structure, mission, vision, philosophy, and values will better prepare nurse leaders to improve quality and patient outcomes.

Identify appropriate outcome measures and study designs applicable to epidemiological subfields such as infectious disease

The purpose of this assignment is to
• Provide learners with the opportunity to integrate knowledge and skills learned throughout this course
• Directly apply principles and knowledge learned in the course to problem solving of population health problems in their own geographic areas.

Course Outcomes
This assignment enables the student to meet the following course outcomes:
1. Define key terms in epidemiology, community health, and population-based research.
2. Compare study designs used for obtaining population health information from surveillance, observation, community, and control trial based research.
3. Identify appropriate outcome measures and study designs applicable to epidemiological subfields such as infectious disease, chronic disease, environmental exposures, reproductive health, and genetics.
4. Apply commonly used measures of health risk.
5. Examine current ethical/legal issues in epidemiology.
6. Identify important sources of epidemiological data.
7. Evaluate a public health problem in terms of magnitude, person, time, and place.

For the final assignment, Milestone 3: Intervention and Evaluation, you will create a PowerPoint. This PowerPoint outlines your intervention and evaluation plan and could be presented to community leaders in hopes of gaining their support to carry out this intervention.3. Be sure to follow the guidelines carefully, and identify your role in this intervention, such as the school nurse, public health nurse, or other community nursing role.4. Begin by introducing the, and provide data that shows why this is concerning in your area. You can use some of the most important data from Milestone 2.Discuss your proposed nursing intervention, including all the points required on the guidelines. Be sure this is something a community health nurse would typically address. This section covers the who, what, how, and where of your intervention. You also need to discuss whether this is primary, secondary, or tertiary prevention.Next, justify why this intervention is important. Discuss information from two scholarly journal articlesthat support this rationale. Cite these references on the slide with author and date, and then include the complete reference on the reference slide.5. In the next section, discuss how you would evaluate this intervention to see if it worked. This section is worth 70 points, so don’t skimp here. Explain what you would measure to show that your intervention is working. Include how and when you would measure these items. Then discuss the short and long term impact of your intervention if it was successful. On a summary slide, review your problem and the intervention, and thank your audience for their support. Finally, include a reference slide with the full references of any sources you cited within the PowerPoint.Remember, PowerPoints should have short bullet phrases, not long sentence or paragraphs. You can add more detail in the speaker notes at the bottom of each slide if necessary, for clarification. Use an interesting colored template, and include graphics, such as clip art or pictures to add interest..

What differentiates the role of the community and public health nurse from other health professionals?

This week, your Instructor assigned you a chapter in the course textbook that discussed the role of the public health nurse in a specific setting.Based on your assigned chapter and bearing in mind the primary prevention focus of the community/public health nurse, respond to the following:

• What differentiates the role of the community and public health nurse from other health professionals?

• What skills and knowledge are necessary for the type of decision making inherent in population-focused nursing practice?

• How do these skills differ from those required in the institutional setting?

Using Chapter 2 in the course textbook, your assigned reading on the role of the nurse in a specific public health setting, and this week’s media presentation, address the following in a 2- to 3-page paper:
Trace the history of the public health nurse including the contributions made by Lillian Wald and the Henry Street Settlement. Also, summarize the role of the nurse in the practice setting assigned. Include the following in your summary:

What is the CH Nurses role

The purpose of this PowerPoint presentation is to provide an opportunity to develop a community health nursing intervention and evaluation tool for your identified community health problem (described in Milestone 2: Assessment and Diagnosis) and identify the components of the nursing process as it applies to a community or population.
Course Outcomes
Scenario
You are a Community/Public Health Nurse (C/PHN) working in your setting of choice. You have identified a community health problem and have analyzed the data collected from your windshield survey and assessment & diagnosis assignments (the first two milestones). You have decided on one nursing intervention and need your organization’s approval for funding of this intervention. Your leadership team has agreed to listen to your proposal.
Directions
• Choose a community health nurse setting. Some examples of settings are school nurse, parish nurse, home health nurse, nurse working in the health department (be specific to what area in the health department, e.g., WIC, STD clinic, health promotion, maternal-child health, etc.)
• Introduction: Introduce the identified problem, the purpose of the presentation, and reiterate at least one or two important findings that demonstrate this problem in your community (average of 1–2 slides)
• Proposed Intervention: Propose one community health nursing intervention that would address one or more of the major direct or indirect factors that contribute to the problem. Keep in mind the Public Health Intervention Wheel (Nies & McEwen, p. 14, figure 1-3) as your framework (average of 3-4 slides). Your intervention needs to be specific:
o Who is your target population?
o Where is this intervention taking place?
o Will it take place one time or multiple times?
o How will you reach out to your target population?
§ How will you get your target population involved?
o What is the CH Nurses role in this intervention?
o Will you collaborate with anyone (e.g., physician’s office, church, local resources, etc.?)
o Is anyone else involved besides yourself (C/PHN)?
§ If yes, are they paid or volunteers?
o What level(s) of prevention is your intervention addressing (primary, secondary and/or tertiary prevention)?
• Intervention Justification: Justify why the problem and your nursing interventions should be a priority.
o Based on what you have found in the literature, discuss why these interventions are expected to be effective. Include summarized information from at least two professional scholarly sources related to your interventions (average of 2-3 slides).
• Proposed Evaluation: Your presentation must include at least one proposed quantitative or qualitative evaluation method that you would use to determine whether your intervention is effective. It should include the method you would use along with desired outcomes. Outcome measurement is a crucial piece when implementing interventions. There is a helpful tool in Doc Sharing to assist you with understanding qualitative and quantitative methods of evaluation. (average of 2-3 slides)
