Hamric’s model of advanced practice nursing was created in 1996 and is composed of seven core components (Hamric, 2014). The three elements of Hamric’s definition of advanced nursing practice that I think are most important are; collaboration, evidence-based practice and leadership.

   Initially, collaboration is important in nursing practice because in our field we have to collaborate with each other every day to give the patient the best care possible. Communicating with the attending doctors, nursing assistants and other staff members can ensure for a more positive outcome for patients. If we don’t work together as a team and collaborate with each other the patients are not going to receive the treatment they need in a timely fashion.

   Secondly, evidence-based practice is also a key element for advanced practice nursing. The three main components of EBP, research, clinical expertise and patient preferences can help improve outcomes (Understanding Evidence-Based Practice, 2016). By following this, proven research allows for a quicker and more affective diagnosis and treatment of patients.

   Lastly, leadership as other elements of Hamric’s models of advanced practice nursing is a crucial piece for advanced nursing. A good leader is focused on what is best for patients, staff and their organization. Doing so will ensure better outcomes and satisfaction in the work place for both patients and staff.

I feel one of the biggest issues is that the new director didn’t include the staff in any input. According to Huber (2014) “power can mean the ability to compel obedience, control, or dominate”. Weber (1947) defined power as “the probability that one actor within a social relationship will be in a position to carry out his own will, despite resistance, and regardless of the basis on which this probability rests” (p. 52). I think if the new director had given some empowerment to the staff then they transition of the new reorganization would go much smoother. Also, if the new director got a group of the co-workers together and actually explained the why’s and how’s of his restructuring, then maybe he would get others on board. It is important to get a group together because group decision making creates a synergy when the group comes together to makes decisions, and acts as a united front. Using the influencing tactic of Ingratiation by “trying to make the other person feel important—giving praise or sympathizing” (Huber, 2014, p161) would also be wise.

(a) What cultural considerations are important for you to remember while you interview Ms. Li? 

When interviewing Ms. Li, one has to clearly understand that her ethnicity is Asian America and there has been a lot of differences in culture that she has lived in the United States of America. The reason for the differences is because as a country, the United States has differences cultural norms that she has lived with during her childhood. Asians are considered conservative hence she should be given an opportunity to choose whether she would prefer a male or female practitioner. It is by giving her a conducive environment that will make her try and be more open towards the doctor who will be interviewing her. Preferably, it may be better if an Asian practitioner would be available as it would make her more open. Considering the age of the patient which is 20 years a lot of resistance is expected, and this may force the practitioner to be sagacious in the word which he or she chooses. The doctor has to take these aspects into considerations so as to ensure that the interactions will be productive and helpful to both parties (Green, 2014).

(b) What is the abuse assessment screen?

 Abuse assessment screen is aimed at detecting any abuse towards women who are pregnant. If any violation is discovered the woman is immediately referred to a counselor who in turn advises the women and encourages them. Consequently, documentation is also done for future medical records.

(c) If abuse is discovered, what should you do?

 If abuse is discovered, as a nurse you have to report it immediately. As nurses, we have a duty to report all cases of suspected abuse. Your healthcare facility likely has a policy and procedure outlining the exact steps you should take. Document exactly what you see and hear from the patient. Contact the appropriate state or county agency. Although the law varies  from state to state, nurses are legally  require to report all cases of suspected abuse to the appropriate authorities. If possible provide the injured party with information and resources. In the case of battered woman that may be the name and  number of women’s  shelter and  domestic violence hotline

Discussion: Improving Cardiorespiratory Fitness

Certain inherited characteristics and behaviors may place people at higher risk of health problems (such as cardiovascular disease, stroke, diabetes, and certain cancers) and death. Physical inactivity doubles the risk for heart disease, while regular activity cuts the risk in half. A growing number of studies shows that regular physical activity also lowers the risk of having a stroke (Do Lee, Folsom, & Blair, 2003).

Physical activity may reduce or eliminate risk factors of high serum cholesterol levels, high blood pressure, glucose intolerance, obesity, and high stress. Moderate physical activity is associated with a reduced risk of heart disease, but the physical activity must be a regular lifestyle habit to have an impact. The greatest benefits occur in sedentary people who adopt moderate physical activity habits. Even as little as 2 miles of brisk walking on most days of the week can be beneficial, while additional protection against heart disease is gained as the time and intensity of exercise increases.

To prepare for this Discussion:

  • Review Chapter 3, “Cardiorespiratory Endurance: Assessment and Prescription” and consider how physical activity benefits cardiorespiratory fitness.

Assignment: Fitness Activity: Developing Cardiorespiratory Fitness

By making some simple changes, everyone can include more physical activity in their daily life. The “lifestyle approach” seeks to increase opportunities for physical activity throughout the daily routine and accumulate at least 30 minutes over the course of most, if not all, days of the week. This is the minimum amount of physical activity that improves the quality of life while decreasing the risk of most chronic diseases. Examples of simple changes that may increase physical activity include:

  • Walk, cycle, jog, or skate to work, school, or the store
  • Take the stairs instead of the elevator or escalator
  • Play sports with the kids

It is important to understand, however, that exercise is not synonymous with physical activity; it is a subcategory. Exercise is physical activity that is planned, structured, repetitive, and purposeful, with the goal of improving or maintaining physical fitness. Through exercise, additional health and fitness benefits can be achieved by adding more time in moderate-intensity activity, or by substituting more vigorous activity.

A “personalized fitness program” should consider the individual’s current activity status and desired outcomes. Cardiorespiratory endurance can be considered the most important of the health-related fitness components because of its impact on the cardio (heart), vascular (blood vessels), respiratory (lungs), and musculoskeletal systems of the body. Cardiorespiratory fitness is a good measure of the heart’s ability to pump oxygen-rich blood to the muscles. The American College of Sports Medicine (Haskell et al., 2007) recommends participating in activities that use 55–90 percent of maximum heart rate for 20–60 minutes, three to five days/week.

To get support and funding for implementations, communication is key. There are several types of communication, including formal and informal types of communication. Communication can be both verbal and non-verbal. It is important for the presenter to not only speak well, but also listen intently when a question is asked, and notice cues from the audience, such as crossed arms, nodding off, or distractions (Huber, 2014).

The communication tool that I would find most helpful with upper management is a power point presentation shown during a meeting. This presentation can cater to several different type of learners. The power point presentation can deliver key information that management needs to know. Some people learn better with visual information while others may understand better with verbal communication. By speaking during the presentation, answering questions, and displaying corresponding data, I may have a better chance at getting support and funding.

Managers are often busy, so how the information is presented is key. Duarte, with Harvard Business Review suggests key points when creating a presentation. First, the presenter should summarize the entire presentation up front, as if their presentation go cut short, and have an overall presentation of no more than 30 minutes. The key points that should be presented are findings, recommendations, conclusions, and calls to action. When these key points are made, the supporting data should be introduced. This is a great tip to remember (Duarte, 2012). As I mentioned before, management has a lot on their plate. They do not always have the time to sit down for an entire meeting. I have witnessed several managers have to walk out on unit meetings halfway through because they had a more important issue to attend to.

Next, Duarte suggests creating summary slides with an overview of key points up front. By having these summary slides, management can ask more questions, and the presenter can show the data within the other slides. They also suggest being quick and direct with your data. Rehearsing before giving the actual presentation is also suggested. You can either do it in front of a mirror or another colleague. A colleague is more appropriate so that they can provide feedback (Duarte, 2012).
